These are the youth consent forms for the 2009-2010 academic year.  If you are new to the youth, or you haven't yet given us one for this year, then you will need to fill one in and return it to:

The details on these forms will be held on a database for administrative purposes only and will not be made available to any third party. The information can be completed by a carer, but only those with parental responsibility can sign the consent. (NB: this may include a foster carer.)
